[Abstract] Non-alcoholic fatty liver disease (NAFLD),has become the most prevalent chronic liver disease globally,and its association with cognitive impairment,dementia,and other brain health isues is receiving increasingattention.In recent years,the liver-gut-brainaxis has emerged as acentral pathophysiological framework,revealing the key mechanisms bywhichgut microbiotadysbiosis,livermetabolicdisorders,systemicinflmmation,andneuroinflammation colectively constitute NAFLD-related encephalopathy. As a first-line treatment strategy for NAFLD,exercise intervention has benefits farbeyond improving intrahepatic lipid deposition, encompasing multi-target regulationof the "liver-gut-brain axis"to exert protective efects on the brain.It summarizes the evidence chain that exercise improves cognitive function through multiple pathways such as reshaping the gut microbiome, enhancing intestinal barier integrity,reducing liver inflammationand insulinresistance,and lowering peripheral and central inflammation levels.Itlooks forward tothefutureclinicaltranslation prospectsandchallenges inthisfeld,includingthedevelopment of precise exercise prescriptions,the aplication of multi omics techniques,and the necesstyof large-scale long-term randomizedcontrolled trials,providingaimportant theoreticalbasisanddirectional guidancefor non-pharmacological intervention of cardiovascular and cerebrovascular comorbidities inNAFLD patients.
