基于标准化随访模式的糖尿病患者自我管理能力提升研究

Abstract：Objective：To evaluate the efectof standardized follow-up model onself-management abilityof diabetic patients.Methods：A total of 200 patients with type 2 diabetes treated in the Department ofEndocrinology of the FirstPeople's Hospital of Baiyin，Gansu Province fromJanuary 2023 to July2O23 wererandomly divided into the observation group and the control group， with 100 cases in each group.The observation group received SFM follow-up management for6 months，and thecontrol group received conventional medical services.HbAlc level，SES score and SDSCA score were compared between the two groups.Results： After 6 months of intervention，HbAlc level decreased significantly in the observation group （P<0.001） ，but itchanged little in thecontrol group （P=0.12 ）.At the same time， the SES score of the observation group was significantly improved （P<0.001） ），which was higher than that of the control group （P=0.06） . In addition， SDsCA scores showed that the behavioral improvement in the observation group was better than that in the control group （P<0.05） .Conclusion：The follow-up management model based on SFM can efectively improve theself-managementabilityof patients with type2diabetesandhelp improve theirblood glucose control status.
