[Abstract]Hypertension is a major public health challenge in China, and its prevalence rate is showing a continuous upward trend. Although clinical blood pressure remains the primarydiagnostic criterion for hypertension at present, nocturnal hypertension has been proven to bea strong predictor of cardiovascular and cerebrovascular events and target organ damage independent of daytime blood pressure. However, its screening is often ignored and the risk of missed diagnosis is high. This review comprehensively presents the research progresson nocturnal hypertension, witha particular focus on thedamage mechanisms and clinical evidence it causes to target organs such as the heart, brain, and kidneys.Ambulatoryblood pressure monitoring is the gold standardfordiagnosing nocturnal hypertension, and the combination of biomarkers (such as the urine albumin/creatinine ratio) helps to identify the risk of nocturnal hypertension atanearlystage.Strengthening theearly identification and managementof nocturnal hypertension isof vital importance for preventing irreversible damage to the heart, brain, and kidneys.
