不同肠外营养时间对早产儿PNAC及BPD发生的影响

[Abstract]Objective: To investigate the effcts of different parenteral nutrition duration on the occurrence of parenteral nutrition-associated cholestasis (PNAC)and bronchopulmonary dysplasia (BPD) in premature infants. Method:The clinical dataof 8O premature infantsadmited to Neonate,Shangrao People's Hospital from January 2023 to December 2O24 were retrospectively colected.All premature infants were uniformly given parenteral nutrition support and were divided into the medium and short-term group ( n =39, <3 weeks) and the longterm group ( n =41,≥3 weeks) according to the duration of parenteral nutrition. The occurrence of PNAC and BPD, and the liver function indicators,kidney function indicators,lipid metabolism indicators,andthe increaseof head circumference and weight were compared between the two groups.Result: The incidences of PNAC and BPD in the medium and short-term group were lower than those in the long-term group ( P <0.05).After the intervention, theliverfunctionindicators,kidneyfunction indicatorsandlipidmetabolismindicatorsofbothgroupsallincreased compared to those before the intervention,the liverfunctionindicators,kidney function indicatorsand lipid metabolism indicators of the medium and short-term group were lower than those of the long-term group ( P <0.05). Three months after the intervention,there were no statistically significant diferences in the increase of head circumference and weight between the two groups of premature infants ( P>0.05 ).Conclusion:Parenteral nutrition for premature infants for along time notonly increases the risk of PNACand BPD,but may alsoaffctliver and kidney functions and induce fat metabolism disorders.
