[Abstract] Objective：To investigate the effcacy and safety of Isotretinoin combined with 30% Salicylic Acid in the treatment of moderate to severe acne.Method： Seventy patients with moderate to severe acne who visited the Department ofDermatological Cosmetologyof Xinyu People's Hospital fromJanuary 2O24 to May 2025 wereselected and divided into a combined group （ n =35，treatedwith Isotretinoin + 30% Salicylic Acid） and a control group （ n =35，treated with Isotretinoin） according to the random number table method.The total effective rate of treatment were compared between the two groups.The global acne grading system （GAGS） score，the level of sebum content ontheface，numerical rating scale （NRS）scoreoffacial aesthetics，and thedermatology lifequality index （DLQI） score were compared between the two groups before and after treatment. The adverse reactions of thetwo groups during treatment were recorded.Result：After6weeks of treatment，the total efective rate in the combined group was higher than that in the control group（ P <0.05）.After6 weeks of treatment，the GAGS score， DLQI score，andthe level of sebum content on the face in the combined group were lower than those in the control group，while the NRS score was higher than that of the control group（ P <0.05）. The incidence rates of variouslocal reactionsanddyslipidemia in thecombined group were higher than those inthecontrol group，butthe differences were not statistically significant （ P>0.05 . No serious systemic adverse reactions occurred in either group.

Conclusion：Isotretinoin combined with 30% Salicylic Acid hasa definiteeffecton the treatment ofmoderate to severe acne，itcan significantly alleviate acne symptoms ofpatients，improvefacial aestheticsandthequalityof life， and does not increase adverse reactions.
