罗哌卡因复合右美托咪定腰方肌阻滞在妇科腹腔镜手术中的应用效果

[Abstract] Objective: To evaluate the effcts of quadratus lumborum block (QLB) with Ropivacaine combined with Dexmedetomidine (DEX)onperioperative hemodynamic stability,analgesic efect,cognitive function, andadverse reactions in patients undergoing gynecological laparoscopic surgery.Method: This study was conducted atFuzhou Matermal and Child Health Hospital fromJune 2022 to April 2025.A total of86 patients who planned to undergo gynecological laparoscopic surgery were included and divided into two groups according to the random number table method. All patients underwent general and QLB. The control group ( n =43)received Ropivacaine for QLB,and the study group ( n =43)received Ropivacaine + DEX for QLB. The MAP, HR, pulse oxygen saturation (SpO2),pain condition (VAS scores),and cognitive function [mini-mental state examination (MMSE)]of the two groups were compared,and the occurrnce of adverse reactions and cognitive dysfunction in both groups were recorded.Result:At1O minutes after anesthesia induction (T1),at the end of skin suturing (T2),and 5 minutes after the surgery (T3) , the MAP and HR of the study group were lower than those of the control group ( P <0.05). The comparison of Sp02 levels at T, T2 and T3 between the two groups showed no statistically significant difference
( P >0.05).At 6,12,24,and 48 hours after the surgery,the VASscores of the study group were significantly lower than those of the control group( P<0.05) .At2,6,12and24 hoursafter the surgery,the comparisonof MMSE scores between the two groups showed no statistically significant difference (P>0.05 ).There wereno statistically significant differences inthe incidence of adverse reactionsandthe incidence of cognitive dysfunction between the two groups P>0.05) . Conclusion: QLB with Ropivacaine combined with DEX during gynecological laparoscopic surgery can significantly alleviate postoperative pain in patients.
