达格列净联合胰岛素强化治疗对初诊T2DM患者胰岛功能及血糖波动参数的影响

[Abstract]Objective: To investigate and analyze the therapeutic eficacy of combining Dapagliflozin with intensive Insulin therapy in patients with newlydiagnosed type2 diabetes melitus (T2DM),as wellas its influence on pancreatic function and blood glucose levels.Method:A total of18O patients with newly diagnosed T2DM admitted to Yuquan District General Hospital,Hohhot City were selected astheresearch objects.The case selection period was from July 2024 to April 2O25.They were divided into the control group (90O cases,with intensive Insulin therapy)and the observationgroup (90 cases,with Dapagliflozintreatmentaddedonthe basis of the control group)by therandom number table method.Both groups were treated continuously for 4 weeks.The blood glucose fluctuation parameters, islet function,oxidative stress level and adverse reactions were observed inthe two groups.Result:After treatment,the postprandial glucose excursion (PPGE) and the largest amplitude of glycemic excursions (LAGE) in the observation group were lower than those in the control group ( P <0.05).After treatment, the homeostasis model assessment of β -cell function (HOMA- β )in the observation group was higher than that control group, the homeostasis model assessment of insulin resistance (HOMA-IR) was lower than that in the control group ( P <0.05). After treatment,malondialdehyde (MDA) in the observation group was lower than that in the control group,and superoxide dismutase (SOD) was higher than that in the control group ( P <0.05). There was no statistically significant difference in the incidence of adverse reactions between the two groups ( P >0.05). Conclusion: The treatment regimen ofcombining Dapagliflozin with intensive Insulin therapycan effectively improve blood glucose fluctuations, enhance pancreatic function,and reduce the levels of oxidative stress in patients newly diagnosed with T2DM. Moreover, it will not increase the incidence of adverse reactions.
