[Abstract] Objective: To investigate the effect of Mycoplasma pneumoniae infection on T helper 17 cell (Th17)/regulatory Tcell (Treg)and levels of myocardial enzymes inchildren with Kawasaki disease.Method: Eighty children with Kawasaki disease who were admitted to from January 2021 to December 2O23 were selected as the case group.Meanwhile,1OO healthy children were selected as the control group. Children in the case group were grouped into infected group and uninfected group according to whether they had Mycoplasma pneumoniae infection. Flow cytometry was used to detect the percentages of Th17 and Treg in peripheral blood. The levels of myocardial enzymes [creatine kinase (CK),creatine kinase-myocardial band (CKMB) and lactate dehydrogenase (LDH)] were measured. The diferences between the case group and the control group, and differences between the uninfected group andthe infected group of the above indexes were compared. Theeffectof Mycoplasma pneumoniae infectionon Th17/Tregand levels of myocardialenzymes inchildren with Kawasaki disease was analyzed.Result: The percentage of Th17,Th17/Treg,and levels of CK,CK-MB and LDH in the case group were higher than those in the control group, the percentage of Treg was lower than that in the control group ( P <0.05). The percentages of Th17, Th17/Treg,and levels of CK, CK-MB and LDH in the infected group were higher than those in the uninfected group, the percentage of Treg was lower than that in the uninfected group ( P <0.05).Conclusion: There exists imbalance between Th17 and Treg in children with Kawasaki disease, and they have more severe myocardial damage than normal children. Mycoplasma pneumoniae infection can exacerbate symptoms of Kawasaki disease, leading to a significant decrease in the percentage of Treg and a significant increase in the percentage of Th17,resulting in a further imbalance in the Th17/Treg ratio.Serum myocardial enzyme levels are also elevated. Clinical monitoring of Th17/Treg and myocardial enzyme profiles can provide insights into the status of immune system dysfunction and myocardial damage.
