[Abstract] Objective：To explore the clinical efficacy of neoadjuvant targeted therapy with Sunitinib before surgeryforlocally advanced renal carcinoma.Method： Atotalof 112 patients with locallyadvanced renal carcinoma admited to Inner Mongolia Hospital of Beijing University Cancer Hospital from July 2O2O to June 2023 were selected for the study and randomly divided into observation group and control group， with 56 cases in each group.The control group underwent nephrectomy，while the observation group was given Sunitinib neoadjuvant targeted therapyfor2cyclesand thenunderwentnephrectomy.Theshort-termeficacy，clinical indicators，tumor markers and survival rates of the two groups were observed.Result：The disease control rate of the observation group was higher than that of the control group （ P- <0.05）. The surgery time and hospital stay of the observation group were shorter than those ofthe control group，the intraoperative blood loss was less than thatof the control group， and the radical resectionratewashigher than that of the control group（ P <0.05）.After treatment， compared with the control group，the levelsof carcinoembryonicantigen （CEA），carbohydrate antigen 125 （CA125），andcytokeratin 19fragment （CYFRA21-1） in the observation group were significantly lower （ P <0.05）. The survival rates of the observation group at6 months and1 year after the surgery were both higherthan those of the control group （ P <0.05）.

Conclusion：Preoperative Sunitinib neoadjuvant targeted therapy for localyadvanced renal carcinoma can improve short-term eficacy，improve clinical indicators，reduce tumor markerlevels，and increase survival rates.
