The Effect of Modified Altemeier Procedure Combined with Autogenous Anal Sphincter Tightening Surgeryon the Treatment ofRectal Prolapse with AnalLaxity/WEILinshaoqiang.//Medical Innovationof China，2025，22（24）： 059-063

[Abstract] Objective：To evaluate the efficacy of the modified perineal-rectosigmoid partial resection （Altemeier） procedure combined with autogenous anal sphincter tightening surgery in the treatment of grade and I rectalprolapse with anal laxity.Method：Aretrospective analysis was conductedon 64 patients with grade and I rectal prolapse withanal laxity who underwent surgical treatment at Sanming First Hospital from June 2O21 to June 2024.The patients were divided into two groups according todiferent surgical methods，the experimental group （32 cases）was treated with the modified Altemeier procedure combined with autogenous anal sphincter tightening surgery，andthecontrol group （32cases）was treated with traditional Altemeier surgery.Theanalfunction，defecation status，anal incontinence score，postoperative complications，and recurrence rate of rectal prolapse were compared between the two groups.Result：At 3 months after surgery，the rectal sensory threshold and maximum tolerance of the experimental group were lower than those of the control group，while the anal canal resting pressure and anal canal maximum systolic pressure were higher than those of the control group，the differences were statistically significant （ P <0.05）.The defecation frequencyof the experimental group was higher than thatof the control group at 3 months after surgery，andtheconstipationscoreandanal incontinence score were lowerthanthoseof thecontrol group，the differenceswere statistically significant （ P <0.05）. The incidence of postoperative complications and recurrence of rectal prolapseintheexperimental groupwerelower than thoseinthecontrol group，thediferences werestatistically significant （ P<0.05 ）.Conclusion： The combination of modified Altemeier procedure and autogenous anal sphincter tightening surgery in the treatment of grade I and II rectal prolapse with anal laxity can effectively improve anal function and reduce the incidence of postoperative complications and recurrence of rectal prolapse.
