[Abstract]Obstructive sleep apnea (OSA) is a common sleep disorder,especially with a high prevalence among bese people.Its pathological mechanism is closely related to intermitent hypoxia and sleep fragmentation. Leptin,a multifunctional hormone secreted byadipocytes,not only regulates energy metabolism but also plays important roles inrespiratorycontrol, inflammatoryresponse,and neuroprotection.Inrecentyears,numerous studies have found that leptin is closely associated with OSA.This review systematicall summarizes the latest research progress on the correlation between leptin and OSA,including the elevation of leptin levels in OSA patients and its interaction with disease severity,gender differences,and obesity.It elucidates the mechanisms of leptin in respiratoryregulatory disorders, neurocognitive impairment,and systemic inflammationactivation.Furthermore,the review discusses the regulatory efects of continuous positive airway pressure (CPAP) therapy on leptin levels and the diferences in individualized responses.The authors suggest thatleptin plays acentral role in the pathogenesis *基金项目:滨州市农社领域科技创新政策引导计划项目(2023SHFZ033)

and progression of OSA,and changes in its levels may serve as a biomarker for predicting the effctiveness of CPAP treatment,ofering new insights for personalized OSA therapy.Future research should focus on elucidating the mechanisms of leptin signaling pathways,developing novel treatment strategies targeting leptin,and advancing precision medicine for OSA.
