[Abstract]Objective: To compare the application effects of posterior lateral approach (PLA)and direct superior approach (DSA) in hip replacement surgery.Method: A retrospective analysis was conducted on the clinical data of238 patients who underwent hipreplacement surgeryin 903 Hospital from June2O18 to June 2022. They were divided into the PLA group (11O cases)and the DSA group (128 cases)according to diferent approaches. The PLA group was treated with PLA hip replacement surgery,and the DSA group was treated with DSA hip replacement surgery.The perioperative indicators,pain degree, hip joint function,inflammatory indicators and complications of the two groups were compared.Result: Compared with the PLA group,the DSA group had less intraoperativebloodloss,longeroperationtime,shorter incisionlengthandearliertime togetoutofbedforactivities 0 P <0.05).At 14 days after the operation,the VAS scores of both groups were lower thanthose before the operation, and the score of theDSA groupwaslower thanthat of thePLA group ( P <0.05).Six months after the operation, the Harris hip function score (Harris sore)and Berg balance scale (BBS)scores ofboth groups were increasedcompared with those before the operation,and the scores of the DSA group were higher than those ofthe PLA group ( P <0.05). Two days after theoperation,the CRPlevels inboth groups were increasedcompared with those before theoperation, but those in the DSA group were lower than those in the PLA group ( P <0.05). The incidence of complications in the DSA group (5.47 % )waslower than that in the PLA group( 14.55% 川 P <0.05). Conclusion: The application of DSA in hipreplacement surgery has more advantages than PLA.Itcan reduce intraoperative blood loss,alleviate postoperative pain,promote hip joint function recovery,has a lighter inflammatory response,and higher safety.
