[Abstract]ObjectiveToinvestigate the level of empathyandthecharacterization dataand assessment methods forbrain functioni patients withadolescent-onset schizophrenia.MethodsTheChineseFacialAfectivePictureTask（CFAPT）was used.Atotalof2Opatientswithfirstepisodeschizophrenia（schzophrenia group）and16healthyvolunteers（ontrolgroup）were enroled，and InterpersonalReactivity Index （IRI），CFAPT，andfunctional magneticresonanceimaging（fMRI） were performed. Thelevels ofempathyandbrainfunctionactivation werecomparedbetweenthe two groups.ResultsCompared with thecontrol group，theschizophrenia grouphadasignificantlylowertotalscoreofempathyandsignificantlylowerscoresofperspectivetaking， fantasy，and sympathetic concern （ Φt=-9.73 to -6.73，P<0.01） ，as well as a significantly higher score of personal distress（ Ψt= 13.17，P<0.01） .Comparison of brainactivationon fMRIinthe context of empathy task between thetwo groups showed that as for positiveemotiontasks，theschizophreniagrouphadsignificantlyloweractivationlevels thanthecontrolgroupintherightinferior parietal lobule and the left posterior central gyrus （t=20.12，19.46，P<0.05） ；as for negative emotion tasks，the schizophrenia group had significantly higheractivationlevelsthan thecontrol groupintheright Rolandicoperculum，theleftsuperiortemporal gyrus（includingtheinsula），therightdorsolateralprefrontalcortex，therightanteriorcentralgyrus，andtherightsuperioremporal gyrus （including the hippocampus） . ConclusionThe emotional symptoms （such as negative symptoms，afectivesymptoms，andthelevelofempathy）andcharacteristicchangeson fMRIshouldbetaken seriouslyinpatients withadolescentonsetshzophrenia，whichprovidesanimportantrferenceforarlydagnosis，differentialiagnosis，andprognos tic prediction.
