[Abstract]Objective：To investigate the relationship between stigma and quality of life of stroke patients with hemiplegia and their caregivers based on the actor-partner interdependence model （APIM）.Method： A total of 207 pairs of stroke patients with hemiplegia and theircaregivers who received treatment in Jiujiang NO.1 People's Hospital from June 2O22 to June 2O24 were selected as the study objects by convenience sampling method. Data were collected using the general dataquestionnaire，the internalized stigma scale of mental ilness （ISMI），the affiliate stigma scale （ASS），and the World Health Organization quality of life brief scale （WHOQOL-BREF）. APIM was constructed by Pearson correlation analysis and Amos 24.O software to examine the actor-partner effect. Result：Inthisresearch，2O7questionnaires weredistributed，withOObeingvalidlyreturmed，yeldinganeective response rate of 96.62% . The ISMI standard score of stroke patients with hemiplegia was （ 2.60±0.51 ）scores， indicatingamoderate level，while theASSstandard score ofcaregiverswas 2.21±0.50 ）scores，indicatingalow to moderate level. The total score of WHOQOL-BREF of the patients was （69.26±8.09 ）points，the total score of WHOQOL-BREF of the their caregivers was 84.18±7.36 ）points.Both the ISMI standard scores of the patients and the ASS standard scores of the caregivers were negatively correlated with the total scores of WHOQOL-BREF （ P- <0.05）.Analysis through the APIMrevealed that the stigma of both patientsand caregiverscoulddetrimentally impact their own quality of life （ β= -8.333， -6.069 ， P <0.05）; stigma also negatively afected the quality of life of the other person（ β=-4.082 ，-3.137， P <0.05）.Conclusion：The stigma associated with stroke patients with hemiplegia and theircaregiversis intimately linkedto the qualityoflifeforboth groups，suggesting that during stroke rehabilitation，attention shouldbe given to thedynamic interplaybetween patients and caregivers.Itisrecommended to deviseacomprehensive psychological intervention program involving both parties to enhancetheir qualityof life.
