支气管肺泡灌洗术联合阿奇霉素与清肺柴胡饮加减治疗小儿肺炎支原体肺炎的效果

[Abstract]Objective:To observe the therapeutic effect bronchoalveolarlavage (BAL) combined with Azithromycin andmodified Qingfei Chaihu Yin in the treatment Mycoplasma pneumoniae pneumonia (MPP) in children.Method: EightychildrenwithMPPwho were admitted toShangrao People's Hospital fromJanuary 2023 to June 2O24 were selected.According to the random numbertable method,they were assgned to the control group and the observation group, with 4O cases in each group.The control group was treated with BAL combined with Azithromycin,whiletheobservation group was treated with modified Qingfei Chaihu Yinonthe basisthe control group. Two groups were treated continuously for two weeks. The serum indicators ( IgA , CRP, IL-6, FIB), lung CTindicators (ground-glass opacity,lithotripsy sign,lung consolidation opacity) beforeand after treatment.The clinical efficacy,the improvement time symptoms in total effective patientand incidence adverse reactions were compared between the two groups.Result: After treatment,the observation group showed significant rises in IgAlevelandclinical efectiverate,andsignificantreductionsinthelevelsCRP,IL-6andFIB,thedetection rates ground glass shadow,lithotripsysign,lung consolidation opacity,andthe improve time each symptom was earlier than that in the control group ( P <0.05).The incidence adverse reactions showed no statistical difference between the two groups ( P>0.05 ).Conclusion:BAL combined with Azithromycinand modified Qingfei Chaihu Yin has aremarkable efect in the treatment MPP in children.It significantly improves biochemical indicators such as immune function and inflammatory factors,as wellas thecondition lung lesions inchildren, and accelerates the disappearance symptoms.

肺炎支原体肺炎(MPP)属于儿童临床常见肺炎类型之一,其主要发病机制即为肺炎支原体(MP)细胞穿过黏液层附着于纤毛上,或通过滑动运动转移至肺泡细胞表面引发支气管炎症进而导致MPP的发生,西医首选大环内酯类抗生素如阿奇霉素进行治疗,但易导致耐药性或多种不良反应的发生[-2],支气管肺泡灌洗术(BAL)随着医疗技术的不断发展也逐渐成为儿科呼吸系统中的常用诊疗工具之一,其主要通过负压将气道内的分泌物清除而缓解堵塞现象[3]。(剩余5969字)
