[Abstract]Objective：To observeand investigate the effect of Paricalcitol combined with Cinacalcet n the treatment ofsecondary hyperparathyroidism （SHPT） in chronic kidney disease.Method： A total of 8O patients with SHPTinchronic kidneydisease diagnosed in Jiujiang Hospital of Traditional Chinese Medicine from January 2022 to January 2O25were selected.The patients were divided into theParicalcitol group andthecombined group by simple random grouping method， with 40 cases in each group.Patients inthe Paricalcitol group were treated with Paricalcitol，and those in the combined group were treated with Cinacalcet combined with Paricalcitol.Both groups received continuous treatment for12 weeks.The clinical eficacy，intact parathyroid hormone （iPTH），parathyroid volume，bone metabolism indicators，bone mineral density，calcium and phosphorus metabolism indicators，and adversereactions ofthe two groups were compared.Result： After 12 weeks of treatment，the total efective rate in the combined group was 92.50% ，significantly higher than 75.00% in the Paricalcitol group （ P <0.05）. After 12 weeks of treatment，the iPTH inthe combined group was lower than thatin the Paricalcitol group，and the parathyroid volumewassmaller than thatin theParicalcitol group （ P <0.05）.After12weeksof treatment，the level ofalkaline phosphatase （ALP）in the combined group was lower than thatinthe Paricalcitol group，andthebone mineral density washigher thanthatintheParicalcitol group （ P <0.05）.After12weeksof treatment，thelevelsofbloodcalcium and blood phosphorus in the combined group were lower than those in the Paricalcitol group，and the product of phosphorus and calcium was lower than that in the Paricalcitol group （P<0.05） .There was no statistically significant difference in the incidence of adverse reactions between the two groups （ P> 0.05）.Conclusion： Paricalcitol combined with Cinacalcet in the treatment of SHPT in chronic kidney disease can effectively reduce iPTH levels，control calciumand phosphorus metabolism disorders，improve bone metabolism，and has significantclinical efectand good safety.
