[Abstract]Objective：To explorethe influenceof Hashimoto's thyroiditis （HT）backgroundon the diagnosis of benignand malignantthyroid nodulesbyultrasound-guided cytological examination.Method：Aretrospective analysis was conducted on8O patients with thyroid nodulesadmited to Ganzhou Cancer Hospital from March 2022 to December 2024.A totalof 34 patients （41 nodules）with HT were set as the HT group，and 46 patients （57 nodules） without HT were set as the non-HT group.All patients underwent ultrasound-guidedcytological examination.The ultrasound-related indicators （nodule size，noduleecho，nodule morphology，nodule boundary，blood flowsignal） and Bethesda clasification of the two groups werecompared.The sensitivity，specificity，negative predictive value， positive predictive value and accuracy of ultrasound-guided cytology in differentiating benign and malignant thyroid nodules were analyzed.Result： Compared with the non-HT group，the proportion of nodules with rich blood flow signal and unclear boundaries in the HT group were higher （ P <0.05）. There was no statistically significant differences in the size，shape and echo of the nodules between the two groups （ P >0.05）. The proportion of type II nodules in the HT group was lower than that in the non-HT group （ P <0.05）. The specificity of ultrasound-guided cytological examination in diagnosing malignant thyroid nodules in the HT group was lower than thatin the nonHT group （ P <0.05）.There were no statistically significant diferences in the sensitivity， negative predictive value， positive predictive value and accuracy between the two groups （P>0.05 ）.Conclusion： Thyroid nodulescombined with HT have rich blood flow and unclear boundaries，which will have a negative impact on the specificity of ultrasound-guided cytology in diferentiating benignand malignant tumors.It is necessary to make acomprehensive judgment in combination with other examination schemes.
