TBIL、GGT及Hb预测高胆红素血症新生儿 并发急性胆红素脑病的价值

[Abstract]Objective: To analyze the value of TBIL, gamma-glutamyl transpeptidase (GGT),and Hb in predicting acute bilirubin encephalopathy (ABE)in neonates with hyperbilirubinemia.Method:Eighty neonates with hyperbilirubinemia admitted to the Shangrao People's Hospital from September 2O21 to September2024 were selected,and divided into group A (without ABE)and group B(with ABE)according to whether ABE occurred during one month of follw-up.Liver function indicators (IBIL,TBIL,GGT,ALT),blood routine indicators[Hband albumin (ALB)],and TBIL/ALB (B/A) values in the two groups were compared. The relationship between above indicatorsand the occurrence of ABE in neonates with hyperbilirubinemia was analyzed.The ROC curves were used to evaluate the predictive value of liver function indicators and blood routine indicators for ABE in neonates withhyperbilirubinemia.Result:Atotal of 31 cases( 38.75% )neonates with hyperbilirubinemiadeveloped ABE. IBIL,TBIL,GGT,ALT, Hb,and B/A in group B were significantly higher than those in group A,ALB was significantly lower than thatin group A,with statistical significance ( P <0.05).Point two-column correlation analysis showed that TBIL,GGTand Hb were positivelycorrelated with ABE in neonates with hyperbilirubinemia ( r>0 , P <0.05).However,there were no significant correlation between IBIL,ALT,ALB,B/A with ABE in neonates with hyperbilirubinemia( P>0.05 ).TheAUC of TBIL,GGT,andHb for predicting ABE inneonates with hyperbilirubinemia were 0.769, 0.683,and 0.657 'P<0.05 ). The AUC of TBIL was the largest. Conclusion:Early TBIL,GGT,and Hb inneonates with hyperbilirubinemia are closely related to theoccurrence of ABE.The three have high predictive value for ABE, especially TBIL.
