[Abstract]Objective： To explore the therapeutic effect microapical surgery combined with bone tissue regeneration surgery in patients with chronic periapical periodontitis.Method： Eighty patients with chronic periapical periodontitis enrolled inthe llfacial Surgery，Tengzhou CentralPeople's HospitalAfiliated to Xuzhou Medical University from December2O22 to December2O23 wereselectedand divided into control group （ n =40）and study group n =40）according to random grouping. The control group was treated with microapical surgery，and the study group was treated with microapical surgery combined with bone tissue regenerationsurgery.The clinical eficacy，inflammatory factor levels，periodontal indices，alveolarbone mineral density and painscore were evaluatedbetween the two groups.Result：The total effectiverate the study group was higher than that the control group （ P <0.05）. TNF -α ， IL-6 and CRP the study group were lower than those the control group1 month after surgery，periodontal pocket depth，tooth looseness index，plaque index，gingival index， gingival sulci bleeding index and loss atachment the study group were better than those the control group at 3 months after surgery， mean gray value X-ray the study group were higher than those the control group 3 months，6 months and 12 monthsafter surgery，the diffrences were statistically significant （ P <0.05）. There were no significant differences in VAS scores between the two groups1 day，3 days and 7 days after surgery （ P >0.05）. Conclusion： Microapical surgery combined with bone tissue regeneration surgery can improve the periodontal index，inflammation level chronic periapical periodontitis patients，andaccelerate the recovery alveolar bone density.
