[Abstract] Objective: To analyze the risk factors for myocardial injury after acute organophosphorus poisoning based on Nomogram.Method:A retrospective analysis was conducted on the clinical data of 92 patients with acute organophosphorus poisoning admited to of from November 2O21 to January 2O24. Based on the results of echocardiography,the patients were divided into a myocardial injury group n =35)and a non-myocardial injury group (n=57).Basicpatient information and treatment details werecollected, including age,gender,BMI,underlying diseases,degree of poisoning,time to gastric lavage initiation,treatment environment,method of airway establishment,and whether blood perfusion was performed. Peripheral venous blood samples within 24 hours of admission were taken to measure serum levels of white blood cell count,plateletcrit,hematocrit,blood calcium,serum creatinine,lipid indicators,and lactatedehydrogenase. Serumlevels of cardiac troponin I, creatine kinase-MB,and plasma N-terminal pro-B-type natriuretic peptide were detected using ELISA.Additionally, the acute physiologyand chronic health evaluation I (APACHEII)score were assessed.logisticregression analysis was used to screen for influencing factors of myocardial injury,a Nomogram prediction model was constructed,and the model's fiting efect was evaluated.Result: Multivariate analysis results showed hatage,severityofpoisoning,timeto gastric lavage initiation,blood perfusionstatus,lactatedehydrogenase, APACHEII score,serum cardiac troponin I, creatine kinase-MB,and plasma N-terminal pro-B-type natriuretic peptidewere influencing factors for myocardial injury in patients with acute organophosphorus poisoning (P<0.05). Thecalibrationcurve indicatedthatthe predicted values ofthis prediction model were close totheidealcurve,the average absolute errorof the calibration curve of the line graph model was O.O21. Conclusion: Advanced age,severe poisoning, delayed gastric lavage,absence of hemoperfusion treatment,APACHEI score and elevated myocardial markers,etc. are influencing factors for myocardial injury following acute organophosphate poisoning.
