早期吞咽功能训练对老年鼻咽癌放疗患者吞咽功能的影响

[Abstract]ObjectiveToexploretheinfluenceofearlyswalowingtraining ontheswallowing functionof elderlypatients withnasopharyngealcarcinoma receivingradiotherapy.MethodsFromJune2019 toSeptember2O24，51elderlypatients withnasopharyngealcarcinomaintheradiotherapydepartmentofChina-JapanUnionhospitalofJilinUniversitywereanalyzedretrospctively.Theyweredivided intoatraininggroup（26cases）andacontrolgroup（25cases）acordingto whethertheyreceivedswallowing functiontraining.Thecontrolgroupreceivedhealtheducationandfolow-upobservation，whilethetraininggroupstartedswallowing function training for 3-6 monthsin the early stage of radiotherapyon this basis.The swallwing function and nutritional status of the twogroupsimmediatelyafterradiotherapyand3monthsafterradiotherapywereanalyzedandcompared.ResultsImmediatelyafterradiotherapyand3monthsafterradiotherapy，thescoresofwaterswallowtest（WST）andvideofluoroscopicswallowingstudy（VFs） in the training group were lower than those in the control group，while the scores of M . D.Anderson dysphagia inventory（MDADI） was higherthan that in the control group，the differences were statistically significant（ P<0.05 ）．Three months after radiotherapy，the scoresofWSTandVFSSofthetraininggroupweresignificantlylowerthanthoseattheendofradiotherapy，andthediferencewasstatistically significant （ P<0.05 ）.However，there was no statistically significant difference in the scores of WSTand VFSSof the control group compared with those at the end of radiotherapy（ P>0.05 ）. The MDADI scale scores of the training group and the control group werehigher than those at the end of radiotherapy，the difference was statistically significant （ P<0.05 ）.Three monthsafter radiotherapy，thescoreoftepatientgeneratedsubjectiveglobalassessmentofthetraininggroupaslowerthanthatintecontrolgroupandat theendofradiotherapy（ P<0.05） .ConclusionSwallowing training for elderly patients with nasopharyngeal carcinoma in the early stage of radiotherapy can significantly improve their swallowing function and nutritional status.
