[Abstract]ObjectiveTo investigate the prognostic valueofsystemic immune-inflammation index (SI)and CRP/Albratio inelderly patientswith acute exacerbation of chronic obstructive pulmonary disease(COPD)complicated with type I respiratory failure.MethodsClinicaldataof196elderlypatientswithacuteexacerbationofCOPDcombinedwithtypeIrespiratoryfailureadmitted to Jinzhou CentralHospitalfrom March2O19to March2O24were selected,andthepatients weredividedintodeath group( n=24 )and survival group ( n=172 )according to their prognosis after 28 days.SI and CRP/Alb ratio were calculated.Logistic multivariate analysis of prognostic factors of acute exacerbation of COPD combined with type I respiratory failure in elderly patients.ROC curve analysis of theprognosticvalueofSIIcombinedwithCRP/AlbratioinacuteexacerbationofCOPDcombinedwithtypeIrespiratoryfailure. ResultsThere were statisticallysignificant diferences inacutephysiologyandchronichealth evaluationI(APACHEI)score, white blood cell(WBC)count,lymphocyte(LY)count,SII,Alb,CRP,and CRP/Albratio between the twogroups( P<0.05) : logistic multivariateanalysisshowedthatAPACHEIsore,WBC,SII,CRP,ndCRP/Albatiowereallrskfctorsfordathinpatients( P<0.05 ),whileLYcountand Albwereprotective factorsfordeathinpatients( P<0.05 ).ROC curve analysis showed that the AUCof SIland CRP/Albratioinpredictigtheprognosticvalueofpatients was0.862(95%CI:0.78~0.946),thesensitvity was 0.792,and thespecificitywasO.826.ConclusionThecombineddetectionofSandCRP/Albratiohashighvalueandacuracyin predicting the prognosis of elderly patients with acute exacerbation of COPD complicated with type I respiratory failure,and can be used as an important indicator to evaluate the prognosis of patients.
