Introduction to ZHOU Zhen's Experience in Syndrome Differentiation and Treatment of Vertigo Based on Theory of Phlegm-Fluid Retention Causing Dizziness

Abstract：This article introduces Professor ZHOU Zhen's clinical experience in syndrome diferentiation and treatmentof vertigo basedon the pathogenesis theoryof phlegm-fluid retentioncausingdizzinessProfessor ZHOU posits thatphlegm-fluid retention constitutesthecore etiologyand pathogensis of vertigo，arising primarilyfrom spleenstomach deficiency and weaknessas well as dietary excess—particularly excessive consumption of raw/cold foodsclosely linked to modern three excesses and threedeficiencies lifestyle paterns.His syndrome-diferentiated approach integrates three diagnostic dimensions，including primary symptoms，constitution and tongue-pulse signs.1）Focus on the primary symptom and identifysubtle pathological changes：The secondary main symptom of dizziness should primarily manifest as nausea and vomiting，while cases caused by phlegm-fluid retention often present with dry vomiting ornausea.2）Differentiate constitution through physical manifestations：Patients typically present withan obese physique bearing puffy flaccidity—apparent obesity with underlying deficiency.3）Analyze tongue-pulse signs：It is to observe pale-red tongue with swolen dry coating and deep-slippery pulse in patients.Guided by the principle that phlegm-fluid is ayin pathogen requiring warm medicinals，Professor ZHOU combines Linggui Zhugan Decoction with Wuling Powder to simultaneously clear phlegm-turbidity， promote urination，and regulate spleen-stomach clear yang. This therapeutic strategy provides pathogenic outlets while protecting healthy qi.
