多模态超声预测乳腺癌腋窝淋巴结转移的临床价值

[Abstract]Objective:Toevaluatethepredictivevalueof multimodalultrasoundforaxillrylymphnodemetastasis(ALNM) inbreastcancer.Methods:Theultrasoundandclinicaldataof92patientswithbreastcancerwereretrospectivelycolected. All patintsreceivedgrayscaleultrasoundcolorDopplerultrasoundandcontrast-enhancedultrasoundpreoperativelyAccodngto the postoperativepathologicalresults,the patientsweredividedinto ALNMgroup(3Ocases)andnon-ALNM group(62cases). Thegrayscaleultrasound,colorDopplerultrasound,contrast-enhancedultrasoundfeaturesbetweenthetwogroupswere compared,andlogisticegressionanalysiswasperformedtodeterminetheindependentpredictorsofALMinbreastcancer. ROCcurvesweredrawn toevaluatethediagnosticperformanceofeachpredictor.Results:Thediferencesinthebloodflow grade,enhancementdirection,adialenhancementatthelesionmarginandfeedingeselswerestatisticallysignificantbetween the two groups (all P<0.05 ).Multivariate logistic regression analysis showed that blood flow grading ofII-Ⅲ ,radial enhancementatthelesionmargin,andfeedingveselsweretheindependentpredictivefactorsforALNMinbreastcancer. ROCcurveanalysisshowedthattheAUCsofaboveindicatorspredicting theALNMwere0.697,0.603,0.665respectively, while the combined AUC was O.837,higher than the single indicator(all P<0.05 ).Conclusions:The blood flow grading of (20 |-|| ,radialenhancementatthe lesion margin,and feeding veselsareindependent predictive factors for ALNMin breast cancer,thecombinationofthethreeindicatorshasahigherpredictivevalue,whichcanprovidemorereferableinformationfor clinical treatment and prognosis evaluation.
