浸润性乳腺癌术前DCE-MRI定量参数及ADC值与术后2年结局的关系

[Abstract]Objective：ToinvestigatetherelationshipbetweenDCE-MRl quantitativeparameters，ADCvalueandthe2-year postoperativeoutcomesinpatientswith invasivebreastcancer.Methods：Aretrospectiveanalysisof316patientswithinvasive breastcancerwhotreatedwith radical surgerywasmade.All patientsunderwent preoperativeDCE-MRIandDWIexaminations toobtain relevant parameters，including volume transfer constant（ Ktrans ），rate constant （Kep）， extravascular extracellulaer volume fraction（Ve）andADC.Aftera2-yearpostoperativefolow-up，thepatients weredividedintoaporprognosisgroup（3Ocases） andagoodprognosis group（286cases）based on their prognosisoutcomes.ThepreoperativeDCE-MRI parameters and ADC valuesof the twogroupswerecompared.Pearsoncorelationwasusedtoanalyze therelationshipbetween thepreoperative DCE-MRI parameters，ADCvaluesandthe2-year postoperativeoutcomes.ROCcurves wereconstructed toasess thepredictive value of the preoperative DCE-MRI parameters and ADC values for the 2-year outcomes.Results： Ktrans，Kep ，and ADCvalues in the poor prognosis group were higher than those in the good prognosis group （all P<0.05 ），while Ve value was not statistical between the two groups（ P>0.05 ）.Pearson correlation test showed that the preoperative Kuns，Kep and ADC values were positively correlated with the postoperative 2-year outcomes （r=0.521，0.584，0.574，all P <0.05）．ROC curve showed that the combination ofpreoperative Ktrans，Kep and ADC values had a good predictive value for the 2-year postoperative outcomes in patientswithinvasivebreastcancer，withanAUCofO.853，andspecificityandsensitivityofO.766andO.800，respectively. Conclusions： The preoperative DCE-MRI parameters Kuns，Kep andADC values of patients with invasive breast cancer are relatedtothepostoperative2-yearoutcome，andthecombineddetectionofthethreeparameterscanhelppredictthepostoperative 2-year outcome.

浸润性乳腺癌呈浸润性生长，常累及淋巴结，是乳腺癌的常见亚型，早期患者术后可长期生存，而中晚期患者即使在手术、化疗等综合治疗后，仍存在复发和转移情况，整体预后不佳[1-2]。（剩余6718字）
