双层探测器光谱CT定量参数鉴别纵隔淋巴结结核与非小细胞肺癌纵隔淋巴结转移瘤的价值

[Abstract]Objective：Toinvestigatethediferencesofquantitativeparametersofdual-layerspectraldetectorCT（SDCT） inmediastinallymphnodetuberculosisandmediastinallymphnodemetastasesofnon-smallcellungcancer.Methods：A retrospectiveanalysiswasmadeon97lymphnodesfrom41patientswithmediastinallymphnodetuberculosis（tuberculosis group）and108lymph nodesfrom48patientswithmediastinallymph nodemetastasisofnon-smallcellungcancer（metastatic tumorgroup）whounderwentthoracic SDCTexaminationand wereclinicallyconfirmed.SDCT imageswerepost-processd，and theparametersweremeasured，including standardisediodinedensity（nID），standardisedefectiveatomicnumber（ nZeff ）and slope of the energy spectrum curve λ40-70 and λ70-100 in the maximal level of the lymph nodes and in the high-density and low-densityareasintheplainarterial，andvenousphases.Andthediferencesinthevaluesofeachparameterofthetwo groupswerecompared.Andthediagnosticexperimentsonindicatorswithstatisticallysignificantdiferencesbetweethetwo groupswereconductedtoevaluatetheirdiferentialdiagnosticvalueforlymphnode tuberculosisandmediastinallymph node metastasis in lung cancer.Results：During the arterial phase，the nlD，nZeff，λ40-70，λ70-100 at the maximal level of lymph nodes， the nll），nZeff and λ70-100 in the high-density area of lymph nodes，and the nZeff in the low-density area of lymphnodes between the two groups were significantly different （all P<0.05 ）. During the venous in the high-density area of lymph nodes between the two groups were significantly different（all P<0.05 ），while there were no statiticallysignificantdiferenceintheparametersatthemaximalleveloflymphnodesorinthelow-densityareaof lymph nodes between the two groups（all P>0.05 ）.Conclusions ：The quantitative parameters， nll），nZeff，λ40-70，λ70-100 inthe arterialphaseandvenousphaseofSDCTarevaluableforthediferentialdiagnosisofmediastinallymphnodetuberculosisand mediastinallymphnodemetastasisfromnon-smal-cellungcancer，whereasthequantitativeparametersintheplainphaseare difficult to diferentiate between the two.
