Value of thyroid fine-needle aspiration eluent versus cytological smear in detecting BRAFv600E mutation WANGYulian,XIANG Longquan,ZHANG Xiangyu,WANG Jigang(Department of Pathology,School of Basic Medicine,Qingdao University,Qing dao 266071,China)
[Abstract]ObjectiveTocompare the DNAqualityofscraped celsobtainedby thyroid fie-needle aspiration (FNA)fresh eluent versus Pap staining smear, as wellas their sensitivity in detecting BRAFv600E mutation,and to determine the optimal method for detecting BRAFv600E mutation using FNA samples.MethodsThis study was conducted among the patients with differentBethesdacategoriesbasedonpreoperativeFNAcytologywhowerediagnosedwithpapilarythyroidcarcinoma(PTC)basedon postoperative histopathology,among whom there were60patients with Bethesda IIPTC,56 patientswith Bethesda VPTC,and 44 patients withBethesda VPTC,andFNA eluent,Papstainingsmear,andsurgical specimens werecolectedfrom allpatients. Sanger sequencing was used to detect BRAFv600E mutation,and the effct of FNA eluent and cytological smear in the diagnosis of (20号 BRAFv600E mutation was compared with the results of BRAFv600E mutation detection in surgical specimens as the gold standard. ResultsTheconcentrationand purityof DNAextractedfromsurgical specimens were significantly higherthanthoseof DNA extracted from FNA eluent and Pap staining smear (Fconcentration=173.98-1176.22,P<0.01;Fpurity=5.99-18.21,P<0.01) (2 BRAFV600E mutation was detected in the PTC surgical specimens of 135 patients (135/160, 84.4% . In the patients with Bethesda (204号 II/V/M PTC,FNA eluent samples had a sensitivity of 96.1% , 95.7% ,and 94.6% , respectively, in detecting BRAFv600E mutation,while Pap staining smear samples had a sensitivity of 94.1% , 93.6% ,and 97.3% ,respectively,both with a specificity of 100‰ .Conclusion It is feasible to detect BRAFv600E mutation using both thyroid FNA eluent and cellsmear samples.
