[Abstract]ObjectiveToinvestigatetheefectsof totalintravenousanesthesia withremimazolamand propofolon postope rativesleepqualityinpatientsundergoing pailessgastrointestinal endoscopy.MethodsAtotalof181patientsundergoing painless gastrointestinalendoscopywereenrolled.Amongthem，92receivedtotalintravenousanesthesia withremimazolam（groupR）， and89 received total intravenousanesthesia with propofol（groupP）.Patients werefollowedupat1dbefore the procedure（ T0 ） and7d（ T1 ）and 30 d （T2 ）after the procedure. Sleep quality was evaluated using the Pittsburgh Sleep Quality Index （PSQI）. ResultsIn group R，the PSQI score at T1 was lower than that at T0（Z=3.84，P<0.01） ，while the PSQI score at T2 was higher than that at T0（Z=2.45，P<0.05） . In group P，there was no significant difference in PSQI scores among T0 ， T1 ，and T2（P> 0.05）.The differences in PSQI score changes from T0 to T1 and from T0 to T2 were not statistically significant between the Rand P groups （ P>0.05） . In group R，the scores of sleep efficiency，sleep disturbance，and daytime dysfunction at T1 were lower than those at T0（Z=2.04-2.92，P<0.05） ；the scores of sleep duration and sleep efficiency at T2 were higher than those at T0（Z= （204 2.92，2.14，P<0.05） .Conclusion Compared with propofol，the use of remifentanil may improve sleep quality within one week after painless gastrointestinal endoscopy but does not improve sleep quality at one month post-procedure.
