[Abstract] Objective Toinvestigatetheeficacyofpercutaneousvertebroplasty （PVP）versuspercutaneouskyphoplasty （PKP）inthetreatmentofosteoporoticvertebralcompresionfractures（OVCF）andtheinfluencingfactorsforpostoperativecomplications. Methods Aprospectivestudywasconductedamong184patientswithOVCFinourhospitalfrom May2021toMay 2023，andtheyweredividedintoPVPgroupandPKPgroupusingarandomnumbertable，with92patientsineachgroup.Postoperativecomplicationsand1-yearmortalityratewerecomparedbetwenthetwogroups，andtheunivariateanalysiswasusedto identifytheinfluencingfactorsforcomplications，whilethemultivariateLogisticregresionmodelwasusedtoanalyzeindependent influencingfactors. Results Therewasnosignificantdiferencein mortalityratebetwenthePKPgroupandthePVPgroup ΔP>0.05Δ ），butthePKPgrouphadasignificantlyloweroveralincidencerateofcomplicationsthanthePVPgroup χ2=13.266 ， P<0.05） ）.Theunivariateanalysisshowedthatthereweresignificantdiferencesbetwenthecomplicationgroupandthenon-complicationgroupintimeofoperation，preoperativebonemineraldensityT-value，degreofvertebralcompresion，vertebralwaldestruction，multiplevertebralfractures，fasciainjury，injectionvolumeofbonecement，preoperativeCob angle，andpreoperative heightofinjuredvertebra （t=3.961-5.765，χ2=6.412-9.518，P<0.05） ）.ThemultivariateLogisticregresionanalysisshowed thatthedegre ofvertebralcompresion 950% ，vertebralwaldestruction，multiplevertebralfractures，injectionvolumeofbone cement，preoperativeCob angle，andpreoperativeheightofinjuredvertebrawereindependentinfluencingfactorsforcomplications Δβ=-0.587 to 2.462，OR=0.556-11.727，P<0.05） ）. Conclusion BothPVPandPKPareefectivetreatmentmethodsfor OVCF，andcomparedwithPVP，PKPcanreducetheincidencerateofcomplicationsinthetreatmentofOVCF.Theindependent influencingfactorsforcomplicationsshouldbecloselymonitoredwhenPVPorPKPisusedforthetreatmentofOVCF.
