[Abstract] Objective ToinvestigatetheasociationbetwentheexpresionofserumFOXO1andHMGA1andthepropertiesofcarotidplaqueinpatientswithischemicstroke（ICS）. Methods Atotalof102ICSpatientswhowereadmitedtoFourth People’sHospitalofLangfangCityfromFebruary2021toFebruary2024wereenroledassubjects，andacordingtotheproperties ofplaque，theyweredividedintostableplaquegroup （controlgroupwith60patients）andvulnerableplaquegroup （observation groupwith42patients）.TheexpresionlevelsofFOXO1andHMGA1inserum weremeasuredforbothgroups，andaretrospectiveanalysiswasperformedfortheirclinicaldata.ThemultivariateLogisticregresionanalysiswasusedtoinvestigatetheinfluencingfactorsforthepropertiesofcarotidplaque，andthereceiveroperatingcharacteristic（ROC）curvewasusedtoinvestigatethe valueofFOXO1andHMGA1inasesingthepropertiesofcarotidplaque. Results Therewerenosignificantdiferencesbetwenthetwogroupsinage，high-densitylipoproteincholesterol，fastingblodglucose，sex，theproportionofpatientsreceiving diagnosisandtreatmentwithin 6-24 hoursafteronset，smokinghistory，drinkinghistory，andthecompositionratiosofcomorbidities（suchasdiabetes，hypertension，andhyperlipidemia） ）.Comparedwiththecontrolgroup，theobservationgroup hadsignificantlyhighertotalcholesterol（TC），triglyceride，low-densitylipoproteincholesterol（LDL-C），glycatedhemoglobin， whiteblodcelcount，andexpresionlevelsofFOXO1andHMGA1inserum ， P<0.05 ）.Themultivariate

LogisticregresionanalysisshowedthatTC ⩾ meanvalue，LDL- ⋅c⩾ meanvalue，serumFOXO1 ⩾10.5μg/L ，and HMGA1⩾113.0ng/L wereinfluencin factorsforthe roertiesofcarotid laue OR= 1.775-3.522，95%CI=1.024-1.781 to 3.079-6.966 ，al P< 0.05）.The ROC curveanalysisshowedthatserum FOXO1and HMGA1aloneorincombinationhadanareaundertheROCcurveof0.765，0.813，and0.927，respectively，inpredictingthepropertiesofcarotidplaqueinICSpatients，andthecombinationofFOXO1andHMGA1hadaspecificityof 85.8% andasensitivityof 85.6% ，witharelativelyhighpredictivebenefit. Conclusion Theexpresionlevelsofserum FOXO1andHMGA1aresignificantlyasociatedwiththepropertiesofvulnerablecarotidplaqueinICSpatients，whichcanbeusedaspotentialbiomarkersforpredictingthepropertiesofcarotidplaque.
