[Abstract] Objective ToinvestigatethevalueofperipheralblodvonWilebrandfactor（vWF，neutrophil-lymphocyteratio（NLR，andoxygenationindex（OIinpredictingacutelunginjuryinpatientswithsepsis Methods Atotalof 8patients withsepsisandacutelunginjurywhoweretreatedinHaianPeople'sHospitalfromJanuary2021toDecember2023wereenroled asobservationgroup，and10 patientswithsepsisalonewereenroledascontrolgroup.Aretrospectiveanalysiswasperformedfor theirclinicaldata，andperipheralblodvWF，NLR，andOIwerecomparedbetwenthetwogroups.Thediferencesinperipheral blodvWF，NLR，andOIwerecomparedbetwenpatientswithdiferentrisklevelsofacutelunginjury，andthevalueofperipheralblod vWF ，NLR，andOIinpredictingacutelunginjuryinsepsiswasanalyzed. Results Comparedwiththecontrolgroup， theobservationgrouphadsignificantlyhighervWF，NLR，andAcutePhysiologyandChronicHealthEvaluationⅡ （APACHE Ⅱ scoreandasignificantlylowerOI to16.030， P<0.001 .Intheobservationgroup，thehigh-riskpatientshadsignificantlyhighervWFandNLRandasignificantlylowerOIthanthelow-riskpatients t=-5.160 to10.172， P<0.001 .Peripheral blodvWFandNLR werepositivelycorelatedwithAPACHE I score，whileOIwasnegativelycorelatedwith APACHE Ⅱ score r=-0.750 to0.801， P<0.05⋅ .PeripheralblodvWF，NLR，andOIusedaloneorincombinationhadanareaunderthe ROCcurveof0.796，0.815，0.843，and0.712，respectively，inpredictingacutelunginjuryinsepsis，andthecombinationofthe threindicatorshadthehihestsensitivit of 88.5% . Conclusion PeriheralblodvWF，NLR，andOIarecorelatedwiththe severityofacutelunginjuryinpatientswithsepsis，andthesethreindicatorsusedaloneorincombinationhaveacertainvaluein predictingacutelunginjuryinpatientswithsepsis.
