[Abstract] Objective ToinvestigatetheeficacyandsafetyofultrasoundcombinedwithX-ray-guidedpercutaneousnephrolithotom （PCNL）inthetreatmentofhorseshoekidne （HSK）stones. Methods Aretrosectiveanalsiswas erformedfor 10 patientswithHSKstoneswhounderwentPCNLsurgeryinXianyangHospitalofYan'anUniversityfromJanuary2021toOctober2023，andacordingtothemethodforguidanceduringsurgery，theyweredividedintoultrasoundgroupwith58patientsand combinedgroupwith42patients.Fortheultrasoundgroup，real-timeultrasoundimagingwasusedtomonitorandguidechanel establishment，andforthecombinedgroup，ultrasoundcombinedwithX-rayimagingwasusedforchanelestablishment.Aretrospectiveanalysiswasperformedfortheclinicaldataofthepatientsinbothgroups，andthetwogroupswerecomparedintermsof theindicatorssuchasthesucesrateofchanelestablishment，perioperativeexamination，postoperativecomplications，andrenal function Results Comparedwiththeultrasoundgroup，thecombinedgrouphadasignificantlyhighersucesrateofirst-time chanelestablishment，asignificantlyles reductioninhemoglobinaftersurgery，asignificantlyshorterlengthofhospitalstay，a significantlylowerserumlevelofblodureanitrogenaftersurgery，andasignificantlygreaterreductionblodureanitrogenafter surgery （χ2=4.345，t=2.491-10.654，P<0.05） ）.Therewerenosignificantdiferencesbetwenthetwogroupsinintraoperative chanelbleding，chanelestablishmenttime，stoneclearancerate，intraoperativeblodlos，theretentiontimeofnephrostomy tube，andendogenouscreatinineclearancerateaftersurgery ΔP>0.05Δ ）.Therewasnosignificantdiferenceintheincidencerateof postoperativecomplicationsbetwenthetwogroups （P>0.05） ）. Conclusion PCNLguidedbyultrasoundcombinedwithX-ray imagingcanfacilitatesmothestablishmentofpercutaneouschanelandrapidrecoveryaftersurgery，withacomparablelithotripsy efecttoultrasoundimaginginthetreatmentofHSKstonesandafavorablesafetyprofile.
