Mechanism of fire needling on regulating pain sensitization in rats with postherpetic neuralgia via TRPV1/PKC-S/p-Akt signaling pathway

[Abstract]ObjectiveToexplore thepotential mechanismofactionoffirenedling therapyonratswith postherpetic neuralgia (PHN) basedon the transient receptor potential vanilloid1 (TRPV1/protein kinase C-δ (PKC- δ VphosphorylatedAktprotein Ψ(p -Akt)signalingpathway.MethodsAPHNratmodelwasestablishedbyintraperitonealinjectionofvaniloidreceptoragonist (resiniferatoxin,RTX).Twentyfoursucessfullymodeledratsereandomlydividedintotemodel group(TX),positiveduggoup (RTX+gabapentin),andfireneedlinggroup(RTX+firenedletherapy),withightratsineachgroup.Anotherighthealthratswere randomlyselectedasthecontrolgroup.Mechanicalpawwithdrawalthreshold(PWTandpawthermalwithdrawallatency(TWL)were employedtoevaluatethedegreeofhyperalgesiainPHNratstreatedwithfireneedlingtherapy.HEstainingwasusedtoassess histopathologicalchangesinthespinaldorsalhornofrats.TUNELstainingwasusedtoexamineapoptosisofGABAergicneuronsin the spinal dorsal horn of rats.ELISA was used to measure the content of tumor necrosis factor- α (TNF- α )and interleukin- 1β (IL1β)inthespinal dorsalhorntissueofrats.Westernblotwasusedtomeasuretheexpresionsofkeyproteins TRPV1,PKC-and p-Aktithespinal dorsal homtissueofratsineach group.ResultsComparedwiththecontrolgroup,themodel groupexhibited lower PWT values (P<0.O5), higher TWL values ( P <0.05), increased quantity of apoptotic GABA cells P<0.05 ),elevatedlevelsofTNF- α (204号 and IL- 1β 1 P <0.05), and increased expressions of TRPV1, PKC-δ, and p-Akt ( P <0.05). Compared with the model group,both the positive drug group and the fire needling group showed increased PWT values ( P⋅ 0.05). Conclusion Fireneedlingtherapyaleviates theinflammatoryresponseofPHNratsbyinhibiting theTRPV1/PKC-8/p-Aktsignaling pathway, thereby relieving pain sensitization in PHN.

带状疱疹后神经痛(postherpetic neuralgia, PHN)是水痘-带状疱疹病毒再激活导致的特征性临床综合征,以神经支配区域簇集性疱疹伴剧烈神经痛为主要表现,其发病机制涉及复杂的神经病理改变,常导致身体功能障碍、情绪障碍等多系统并发症,严重影响患者的生存质量[I-2]。(剩余16700字)
