[Abstract]ObjectiveToinvestigatetheefectoftheangiotensinreceptorneprilysininhibitor（ARNI）sacubitril/valsartan combinedwithdapagliflozininthetreatmentofacutedecompensated heart failure（ADHF）.MethodsAtotalof104 ADHFpatients whowereadmitedto QinghaiRed Cross Hospital from February 2022toNovember 2023 wereenroledassubjects，anda retrospectiveanalysiswasperformedfortheirclinicaldata.Accordingtothetreatment method，thepatientsweredivided into ARNItreatment group（control group with 50patients）and ARNI+dapagliflozin treatment group（bservation group with 54 patients），andthetwogroups werecomparedintermsofclinicaloutcome，cardiacfunction，serologicalexamination，renalfunction， andadversereactions.ResultsTheobservation grouphadasignificantlyhigher overallresponserate than thecontrol group （χ2=4.479，P<0.05） . Compared with the control group，the observation group had a significantly higher left ventricular ejection fractionandsignficantlylowerleftventrcularinternaldiameteratend-systole，leftventricularinteraldiameteratend-diastole， and left atrial diameter，as wellas significantly greater changes in these indicators after treatment （t=2.700-21.096，P<0.05） ： Compared withthecontrolgroup，theobservationgrouphadsignificantlylowerlevelsofcardiactroponinT，N-terminalprobrain natriuretic peptide，and creatine kinase-MB，as wellas significantly greater changes in these indicators after treatment（ 49.631，P<0.05） . Compared with the control group，the observation group had significantly lower levels of blood urea nitrogen and creatinineandasignificantlyhigherestimatedglomerularfiltrationrate，aswellassignificantlygreaterchangesinteseindicators after treatment （t=4.674-17.131，P<0.05） . The observation group had a higher incidence rate of adverse reactions than the control group （ 7.4% Us 6.0% ， χ2=0.082，P>0.05） .ConclusionARNI combined with dapagliflozin in the treatment of ADHF canimprovetheclinicaloutcomeandcardiacfunctionofpatientsandpromotetherecoveryofrenalfunction，withagoodsafetypofl.
