[Abstract]ObjectiveTo investigatetheefectof perforatedKirschner wire tensionbandonelderlyand midle-aged patients withosteoporoticolecranonfractures.MethodsAtotalof79midleagedandelderlypatientswithosteoporoticolecranonfrac tures who werediagnosedandtreatedin Suzhou High Tech ZonePeople's HospitalfromJanuary 2O2OtoJune2022 wereenrolled as subjects，andaccordingtothemethodforinternalfixation，thepatients weredividedintoobservationgroupwith42patientsand controlgroupwith37patients.ThepatientsintheobservationgroupreceivedinternalfixationwithaperforatedKirschnerwiretensionband，whilethoseinthecontrol groupunderwentanatomical plateinternalfixationfortheolecranon.Thetwogroups were comparedin termsof surgical indicators，elbow joint function （Mayo Elbow Performance Score），Visual Analogue Scale（VAS） painscore，ActivitiesofDailyLiving（ADL）score，andtheincidenceratesofcomplications.ResultsCompared withthecontrol group，theobservationgrouphadsignificantlyshortertimeofoperationandtimetofracturehealingandasignificantlylowerintraoperative blood loss （t=3.437-7.511，P<0.05） . There were no significant differences between the two groups in Mayo Elbow Performance Score，VAS score，and ADL score before surgery（ P>0.05 ）.After surgery，both groups had significant increases in MayoElbow Performance Score andADL score，andtheobservation group had significantly greaterincreasesthan thecontrol group （t=3.554-9.874，P<0.05） . After surgery，both groups had a significant reduction in VAS score（ F=87.978，114.245，P< 0.05），and although the observation group had a lower incidence rate of complications than the control group （4.8% us 10.8% ）， therewerenosignificant diferences inVAS scoreandtheincidencerateofcomplications betweenthetwogroupsaftersurgery 0 ⟨P>0.05⟩ .ConclusionInternalfixation with perforated Kirschnerwiretensionbandcan effectivelyshortenthetimeof operationand thetimetofracturehealing，reduce intraoperativebloodlossimproveelbowjointfunction，aleviatepain，andstrengthen activities nf dailv living
