胃癌病人术后核心症状的识别及影响因素分析

AbstractObjective：Toinvestigatetheincidenceandseverityofpostoperativesymptomsingastriccancerpatients.Toexplorethe network characteristicsandcoresymptomsofpostoperativesymptomsingastriccancerpatientsusing networkaalysismethods，and to analyzethe influencingfactorsofcoresymptoms.Methods：Across-sectionalsurveywasadopted.Atotalof269 postoperativegastric cancerpatientshowerehospitalizedinthedepartmentofgastroenterologyatatertiarygadeAancerspeciisthospitalinHangou cityfromJanuaryO24toAprlO24 wereselectedastheresearchsubjectsconveniently.TheChineseversionofMDAnderson Syptom InventoryGastrointestinal Cancer Modulewasusedforthesurvey.TheRsoftwarewasusedtoconductanetwork analysisof the postoperativesymptoms ingastriccancerpatientsanddeterminethecoresymptoms.Logisticregresionwasusedtoanalyzethe influencingfactorsofthecoresymptoms.Results：Thesymptomswiththehighest incidenceamonggastriccancerpatintsaftersurgery were restlessness during sleep （85.9%） ，fatigue （85.1% ），andpain （83.3% ）.The symptom network had good accuracy and stability.The predictable values of the nodes ranged from 13.1% to 60.0% Distressshowed thegreatest predictability.Vomiting Δrs=2.424 ，fatigue rs= 1.159），and distress rs=0.780 ）were the three nodes with the largest strength centrality and were the core symptoms.Age，gender，and the surgicalmethodwerethemaininfluencingfactorsfortheoccurrenceofvomitig.ThescoresoftheNutritionalRiskScrening202 （NRS2O02），emoglobincount，andpre-albumincountweretheinfluencingfactorsforthecurrenceoffatigue.Educationbackgound andtheoureceofpostoperativecomplicatiosasteiueningfactrfortheurrenceofdistress.Conusis：Tisstudyused thenetworkanalsismethodtoexploretheostoperativesymptomnetworkofgastriccancerpatients，ndfoundthatvomitingfatigue， anddistresswerethecoresymptomsofgastriccancerpatientsaftersurgery.Itissuggestedtatmdicalstaffsouldidentifytecore symptomsasearlyasposible，severtherelationshipsbetweensymptoms，andtakepredictiveinterventionsforthemain influencing factors of the core symptoms to improve the accuracy and eficiency of symptom management.
