Abstract：This article introduces Professor WANG Bangcai's clinical experience in treating refractory cranial tinnitus bysyndromedifferentiation.Professor WANGholds thatthecorepathogenesisofrefractorycranialtinitus is insufficiencyofqiin theupperregioncaused bydepletionanddeficiencyof visceraandbowels，andpathological factors of wind，phlegmand bloodstasisareimportantlinksdisturbingtherestlessofbrainandspirit.The brainandspiritcan be nourished，and the cranial tinnituswill resolve spontaneously after theqi in the upper region is nourishedand pathogens are eliminated.This is achived through five mechanisms，including boosting kidney and supplementing essentialqi to tonifymarrow，calmingheartand spirit tofortifybrain，supplementing spleenand upbearing theclearto tonifyqi，soothing the liverand regulating emotions torestore orifice patency，anddispelling windand resolving phlegm to disperse blood stasis.
