Ll Weilin's Experience in Treating Pulmonary Nodules Based on the "Left Ascending and Right Descending" Theory

Abstract：This article introduces ProfessorLI Weilin's clinical experience in treating pulmonary nodules.Given thediagnosticchallenges in diferentiating benign from malignant nodulesand therequired long-term follow-up， pulmonarynodulespresent significant clinical dificulties.Professor LI Weilin views thisconditionas deficiency interweaved with excess syndrome，characterized by the accumulation of phlegm，blood stasis，and toxins in the lung colaterals.The treatment approach should simultaneously address phlegm resolution，stasis removal，detoxification， deficiencysuppementation，andcolateral unblocking to eliminate pathogens.Thecore therapeutic principle follows the "left ascending and rightdescending"movementofqi，blood，yin，and yang，whileregulating theliver，spleen，and lungs tounblock lungcollateralsand expel pathogens.The"leftascending andright descending"theory posits hat the liver stores bloodand governs ascending movement （yang leading yin）topropel blood with qi，while the lungs govern qi and control descending movement（yinleading yang）to directturbid yindownward.Yinandyang circulate throughout the body with yang qi ascending leftward and yin qi descending rightward.Basedon this theory，Professor LI Weilin modifies clasical formulas including Xiao Chaihu Decoction，Guizhi Decoction，Shengjiang Powder，Biejia Jian Pils， andErchen Decoction.These prescriptions combine ascending and descending herbs to promote yang ascent and yin descent，restore internal-external harmony，guide phlegm-stasis-toxin elimination throughqi movement，expelling pathogens to the exterior，and unblock lung collaterals，thereby pulmonary nodule can dissipate.
