Diagnostic value of exosomal miR-155 and its association with immune-inflammatory status in breast cancerpatients*

[Abstract]Objectie:ToinvestigatethexpressionlevelofexosomalmicroRNA-55(miR-155)intheperipheralbloodofbreast cancerpatients,itsassociationwithimmunefunctionandinflammatoryfactors,andtoevaluate itsdiagnosticvalue.Methods: Peripheralbloodsamples werecollctedfrom102breastcancerpatientsand51healthyfemales(control group).Exosomal miR155and tumor markerCA153 levels were measured.Immunecell subsetsandserum inflammatorycytokineswereanalyzed. DiagnosticeficacyasassessedusingROCcurves.Inadition,Tcellsweretransfectedinvitrotoverifytheregulatorymehanism of miR-155.Results:Exosomal miR-155 was significantly elevated in breast cancer patients( P< 0.05)and was closely associated with lymph node metastasis,distant metastasis,advanced stage,and triple-negative subtype( Pe <0.01).Patients exhibited a decreased CD4+/CD8 + ratio and natural killer(NK) cell activity,increased regulatory T cell(Treg) proportion,elevated IL-6,IL-10,and TNF- ∇⋅α∝ levels,and reduced IFN- γ levels( P- <0.05).miR-155waspositivelycorrelated with Treg,IL-6,IL-10, and TNF- α ,while negatively correlated with CD4+/CD8 + ratio,NK activity,and IFN- γ .ROC analysis showed that miR-155 hadanAUCof0.802forbreastcancer diagnosis,higherthan CA153(O.723).Combined detectionofmiR-155and CA153 achievedanAUCofO.843with improvedsensitivityandspecificity.InvitroexperimentsindicatedthatmiR-155promoted IL-6secretionbyinhibitingsuppresorofcytokinesignaling1.Conclusion:ExosomalmiR-155ishighlyexpressedinbreast cancer.Itscombined detectionwithCA153 enhances diagnosticaccuracy,suggesting itspotentialasa liqud biopsybiomarker for early screening and personalized therapy.
