[Abstract]ObjectiveTo compare the clinical eficacyof unilateral biplane endoscopy（UBE）and percutaneous endoscopiclumbardiscectomy（PELD）fornucleotomyinthetreatmentofsevereprolapsedlumbardischerniation（LDH）inelderlypatients.

MethodsAtotalof8elderlypatientswithsevereproapsedLDH，whoweretreatedatthe9O4thHospitaloftheJointLogisticSpport ForceofthePeople’sLiberationArmyofChinabetweenMay2O2landMay2O24，wereselectedasthestudysubjects.Theywererandomly dividedintotwogroups（UBEandPELD）usingarandomnumbertablemethod，with44patients ineachgroup.Tepaientsinbothgroups underwentnucleotoyunderUBEorELD，espectivelyesurgery-relatedinicators，ospitalizatiouration，ainlevelsolica tions，lumbarfunction，treatmentoutcomesndqaltyoflifewerecomparedndnalyzdesultsComparedithteEgroup，the UBE group had fewer fluoroscopy instances，longer surgical time，and fewer complications （ P<0.05 ）. At 1，3，and 6 months post-surgery， the Visualanalogue scale（VAS）scoresforbackandleg pain inboth groups graduallydecreased compared to pre-surgerylevels （ P<0.05 ）， and the VAS scores in the UBE group were lower than in the PELD group （ P<0.05 ）. There was no statistically significant difference in clinical efficacy between the two groups （ P>0.05 ）.At 3，6，and 12 months post-surgery，both groups showed a decrease in Oswestry disability index（ODI）scoresandanincreaseinlumbarJapaneseOrthopaedicAsociationscore（JOA）scorescompared topre-surgery（ P<0.05 ） The UBEgroup had lower ODI scores and higher JOA scoresat 3and6 months post-surgerycompared tothe PELD group（ P<0.05 ）.Both groups had increased 36-item Short Form Health Survey Questionnaire scores post-surgery compared to pre-surgery （ P<0.05 ）.Conclusion Both UBEandPELnucleotoyarefectiveintreatingelderlypatientswithsevereprolapsedLDH.Hwever，comparedtoELDUBEis more effective in alleviating pain and promoting early lumbar function recovery.
