[Abstract]ObjectiveTo explore the efectof preservingthepubicprostatoligament （PPLs）during surgeryon postoperativebladderfunctionandurinaryincontinenceinelderlypatientsundergoingrobot-assistedlaparoscopicradicalprostatectomy （RALP）.MethodsAtotalof 8Oelderlypatients with prostatecancerwhoreceivedRALPtreatmentinthethe96Oth Hospitalof the JointLogisticsSupportForceoftheChinesePeople'sLiberationfromJune2O19toJune223wereretrospectivecollctedTeywere dividedinto a groupA（37cases，with puboprostaticLigament preserved during RALPsurgery）anda groupB（43cases，with standardRAPsurgery）acoding todiferentsugicalplans.heperioperativeelatedindicators，ostoperativebladderfunctioncidence andseverityofurinaryincontinence，sexualfunction，andqualityoflifebetweenthetwogroupserecolctedadcomparedResults The total hospitalization time of group A was shorter than that of group B （ P<0.05 ）. The differences in the operation time，intraoperativebloodloss，postoperativedrainagetubeindwellingtieandpostoperativecatheterindwellngtimebetweentetogroupshadno statistical significance（ P>0.05 ）．At6 months follow-up，the maximum urinary flow rate，maximum urethral pressure，and bladder compliance in group A were higherthan those in group Bandtheresidual urine volume was lower than thatin group B（ P<0.05 ），the incidenceofurinaryincontinenceingroupAwaslowerthanthatingroupB，andtheseverityofurinaryincontinencewasmilderin group A than in group B （ P<0.05 ），the scores of group A on the international inventory of erectile function-5，Chinese premature ejaculationpatientsexualfuctionevaluationscale-1O，andWorldHealthOrganzationqualityoflifesessmentbriefwerealligher than those of group B （ P<0.05 ）.ConclusionRetaining puboprostatic ligament during surgery in elderly patients with RALP can improvepostoperativebladderfunctionreducetheincidenceofuinaryincontinence，improvesexualfunctionadqualityoflife，and promote recovery.
