[Abstract]ObjectiveTo explore thedetection efect metagenomic next-generationsequencing(mNGS)technologyon elderlypulmonaryinfections.MethodsAtotal289elderlypatientsagedover6OyearswithclinicallysuspectedpulmonaryinfectionsadmitedtotheRegionfromJuly2O21toDecember2O23wereselectedasstudy subjects.AllpatitsuderentmNGohosopitrchalaspiatesandconvtioalmicrobilgicaltestsedagoicpe formancemNGandconventionalmicrobiologicaltestswascomparedusinglinicaldiagnosticresultsasthegoldtandardResults Elderlypulmonaryinfectioswerpredominantlyolcrobial,accoutigfor4.5%(117/89).ediagosticaccuracyadsitivity mNGS were 78.5% and 89.2% respectively,surpassing those conventional microbiological test ( 37.4% diagnostic accuracy, 28.8% sensitivity).Forthe majoritypathogens,thedetectionrate mNGSwashigherthanthatconventionalmicrobiological test.The polymicrobial infectionrate inimmunocompromisedpatientswas73.1%(38/52).Thepathogenspectrumwasalsoobserved tovaryamongdiferentpopulations.Thepositivepredictivevaluewasfoundtodependonthetypepathogenandthepatient'smmune status.ConclusionInelderlypatientswithpulmonaryinfections,polymicrobialinfectionsarethemostcommon.mNGScangreatly improvetheaccuracyanddetectionatepathogensinelderlypatientswithpulmonaryinfections.Inaddition,wheninterpreting mNGSresults,thecomorbiditiesandtypespathogensshouldbeconsidered.mNGShasahighclinicalapplicationvalueinhediagnosis pulmonary infections in elderly patients.
