非酒精性脂肪性肝病患者葡萄糖代谢与肝酶水平的变化及其临床意义

[Abstract]Objective:To investigate the changes of glucose metabolism and hepatic enzyme levels in patients with non-alcoholic fattyliverdisease (NAFLD)and its clinical significane.Method: A totalof 1O6 patients with NAFLDadmited to Jiujiang Third People's Hospital from August 2O2 to July2O24 were selected as the observation group,and 71 healthy individuals who underwent physical examinations during the same period were selected as thecontrol group.The levels of glucose metabolism indicators (fasting blood glucose,C-peptide,glycated hemoglobin)and hepatic enzyme indicators (ALT,AST,gamma glutamyltransferase)were compared between the two groups.Patients inthe observation group were classfied into mild,moderateandsevere fatty liverbasedonthe results of liver color Doppler ultrasound examination.The levels of glucose metabolism and liver enzyme indicators of patients with diffrent degrees offatt liver were compared.The value of glucose metabolism and liver enzyme indicators in the diagnosis of NAFLDwere analyzed.Result:The levelsof glucose metabolismand liverenzyme indicators in the control group were better than those in the observation group ( P <0.05). The results of liver color Doppler ultrasound examinationshowed thatthere were15cases ofsevere fattyliver,31cases of moderate fatty liver and 6O cases of mild faty liver in the observation group.Glucose metabolismand liver enzyme indicators gradually increasedwiththeseverity of fattyliver ( P <0.05).The combined detection of glucose metabolism and liver enzyme indicators has the highest value in the diagnosis of NAFLD, with AUC of O.984, sensitivity of 97.2% ,and specificity of 98.6% .Conclusion: Glucose metabolism and liver enzyme levels are closely related to NAFLD,and increase with the aggravation of thedisease.The combined diagnostic eficacyof glucose metabolismand liver enzyme indicators for NAFLD is relatively high.
