[Abstract]Objective: To investigate the application effect of direct anterior approach and posterolateral approach for total hiparthroplasty (THA).Method: A total of8O patients who underwentelective THA in the Second People'sHospitalof Jingdezhen from January2O21 toMarch2O24 were selectedand divided into directanterior approach group (n=40)and posterolateral approach group (n=40)according to random number table method.The direct anterior approach group underwent surgery through direct anterior approach and the posterolateral approach group underwent surgery through posterolateral approach.Perioperative related indexes,pain degree,hip function and quality of life were compared between the two groups.Result: Compared with the posterolateral approach group,the intraoperativeblood loss in the direct anterior approach group was less,the time ofirst geting outof bed was earlier,and the total hospital stay was shorter ( P <0.05). There was no significant difference in operation time between the two groups ( P >0.05).The numerical rating scale (NRS) scores of the direct anterior approach group 6 and 24 hours after surgery were lower than those of the posterolateral approach group ( P <0.05). The Harris hip scores (HHS)3 and6months aftersurgery inthe direct anterior approach group were higher thanthose in the posterolateral approach group ( P <0.05).Six months after surgery,the scores of each dimension of SF-36 in the direct anterior approach group were higher than those in the posterolateral approach group ( P <0.05). Conclusion: Compared with theposterolateral approach,the direct anterior approach for THA can shorten the rehabilitation proces,reduce postoperative pain degree,promote the recovery of hip function and improve the quality of life.
