[Abstract]ObjectiveToinvestigate therelationship betwenapatent foramenovale(PFO)and cerebral hemodynamics in cryptogenic stroke(CS).MethodsWeincluded 246patients withCSadmittedtotheNeurology DepartmentofTheAfiliated Hospital of Qingdao University from May 2o16 to December 2023.The patients were divided into CS-PFO0 +) group(121 cases) and CS-PFO (-) group(125 cases).Based on the presence or absence of a left-to-right shunt (LRS),the patients in the CS-PFO (20号 (+) group were divided intoLRS group(45cases)andNO-LRSgroup(76 cases).Cerebrovascularrisk factorsand cerebralhemodynamic indicatorswerecomparedacrossdiferentgroups,andtheriskfactors forstrokeinpatientswithPFOwereanalyzed. ResultsThe CS-PFO(+) group showed a significantly higher pulsatility index (PI; t=3.849,P<0.05) and a significantly lower end-diastolic velocity (EDV; t=2.578,P<0.05 )than the CS-PFO (-) group. PI and EDV differed significantly across the CS(2号 PFO(-) group,LRS group,and NO-LRS group ( F=10.584,4.169 ,both ). The PI of the LRS group was significantly higher than those of the NO-LRS group and CS-PFO(-)group(both P<0.05) . The EDV of the LRS group was significantly lower than that of the CS-PFO(-)group( P<0.05) .Anincreased PI was an independent risk factor for stroke in patients with PFO ⟨OR=1.383,P<0.05) .Conclusion Cerebral hemodynamics are impaired in patients with CS complicated by PFO,and LRScausedbyPFOisasociatedwithcerebralhemodynamic impairment.AnincreasedPIisanindependentriskfactorforstroke in patients with PFO.
