[Abstract]ObjectiveToinvestigatetheeficacyofbiolimusA9-coatedbaloons（BCB）versuspaclitaxel-coatedballons （PCB） intreatmentofprimarysmallvesellesions inelderlypatients withunstableangina.MethodsAtotalof160elderlypatients withunstableanginawhowereadmitedtoWeifangYiduCentralHospitalfromApril toOctober 2023andmettheinclusion criteria wereenrolledanddividedintostudy group（receiving BCBtreatmentalone）andcontrolgroup（receivingPCBtreatment alone）using thesimplerandomizationmethod，with 80 patients ineach group.The two groupswerecompared in terms of minimumlumendiameterof the target vesel，degree oflesion stenosis，late lumen loss，andmajoradverse cardiovascularevents （MACE）at9 months after surgery.ResultsCompared with thecontrolgroupat9monthsafter surgery，the study group had a significantly larger minimum lumen diameter of the target vessel Δt=3.525，P<0.001） ，a significantly lower degree of lesion stenosis （t=-4.042，P<0.001） ，asignificantly lower degree of late lumen loss （t=-2.643，P<0.05） ，and a significantly lower incidence rate of MACE ⟨χ2=22.514，P<0.001） ，aswell asa significantly higher MACE-free survival rate（ χ2=21.547 ， P< 0.001）．ConclusionBCBhasbetereficacyandsafetythanPCB inthetreatmentof primarysmallvesellesions inelderlypatients with unstable angina.
