[Abstract]ObjectiveToinvestigate thevalueofconventional whole-body18F-FDGPET/CTlarge jointimagingindisease assessment of rheumatoid arthritis （RA）. MethodsA retrospective analysis was performed for the 18 F-FDG PET/CT imaging dataand clinicaldataof19 patients who werediagnosed withRAfromOctober 2017 toOctober 2O20.Maximum standard uptake value（ SUVmax ）was measured for the bilateral shoulder，elbow，hip，and sternoclavicular joints and the atlanto-axial joint（9 jointsintotal），andthevisualscoreofradioactiveuptakewasdeterminedtoanalyzetheasociationofradioactiveuptakewithdiseasestage，clinical manifestation，andserumC-reactiveprotein（CRP）level.ResultsCompared withthepatientsintheremission stage，the patients in the active stage had significantly higher number of joints with a visual score of ⩾2 ，total visual score， and total SUVmax （ ：t=-2.17，-2.46，z=-2.30，P<0.05）， .The total visual score of the joint was significantly correlated with CRP level （r=0.58，P<0.01） ），and total SUVmax was not significantly correlated with CRP level r=0.35，P>0.05） . The visual score of the atlanto-axial joint was significantly correlated with total visual score （ r=0.65，P<0.05 ）.The number of painful or swollen joints was significantly correlated with the number of joints with a visual score of ⩾2 .Conclusion Patients with RA should undergo conventional whole-body 18F -FDG PET/CT imaging，and large joint imaging can not only sensitivelydetecttheinvolvementoflargejointssuchasthecervicalspine，butalsoefectivelyevaluatethelocalactivejointlesionsof patientsintheremissonstageofRA，therebyshowinganimportantvalueintheclinicaldiagnosis，treatment，andasessmentofRA.
