[Abstract]ObjectiveToinvestigate the efectof minimalpars plana vitrectomy（PPV）and preoperative pan-retinal photo coagulation（PRP）inthetreatmentofsevereproliferativediabetesretinopathy（PDR）．MethodsAtotalof120patients with severePDRtreatedatZhengzhouUniversityFirstAfiliated HospitalbetwenJanuary202landSeptember2O23wereenrolledand randomlyassigned using stratifiedrandomization intoacontrol groupanda studygroup，with6Opatients ineach group.TecontrolgroupunderwenttraditionalPPVcombined with preoperativePRP，while thestudy groupunderwent minimalquantitativePPV combined withpreoperativePRP.Clinicaleficacy，retinalthickness，bestcorctedvisualacuity（BCVA），vascularendothelial growth factor（VEGF）levels，and complications werecompared betweenthetwo groups of patients.ResultsThe total clinical effective rate of the study group （88.33% ） was significantly higher than the control group （66.67% ） ⟨χ2=8.076，P<0.05⟩ . Comparedwiththe preoperativeperiod，bothgroupsshowedsignificantdecreases inretinal thickness，increases inBCVA，anddecreases in VEGF at different postoperative time points （ （F=174.433-630.361，P<0.05） . Compared with the control group，the study groupshowedsignificantdecreasesinretinal thickness，increasesinBCVA，anddecreasesin VEGFat1，3，and6monthsposto peratively Δ′F=28.470-115.819，P<0.05） .The total incidence of complications was 25.00% in the control group and 16.67% in the study group，with no significant difference （ P>0.05） .Conclusion Compared to traditional treatments，minimal quantitativePPVcombinedwith preoperativePRPprovidessuperiorclinicaloutcomes forpatients with severePDR，reducesretinalthickness and VEGF levels，and improves vision.
