Predictive value of pan⁃immune inflammation value for the frailty of elderly patients with colorectal cancer before operation

Abstract  Objective：To analyze the status quo and influencing factors of frailty in elderly patients with colorectal cancer before operation， and to explore the predictive value of pan-immune inflammation value（PIV）.Methods：A convenience sample of 277 elderly colorectal cancer patients was recruited from the departments of general surgery and oncology at a tertiary grade A hospital in Shanxi province between December 2022 and December 2023，and the data were collected using the questionnaire of general demographic data and clinical data， Frailty Phenotype， Health Literacy Scale for Chronic Patients， Quality of Life Instruments for Cancer Patients Colorectal Cancer.Logistic regression analysis were carried out to identify the influencing factors of frailty in senile colorectal cancer patients before operation， and the predictive value of PIV was analyzed by ROC curve.Results：A total of 277 elderly patients with colorectal cancer were included， of which 39.4% were patients with frailty. The results of binary Logistic regression showed that age， gender， BMI， albumin level， PIV， quality of life and health literacy of colorectal cancer patients were the influencing factors of frailty in senile patients with colorectal cancer before operation （P<0.05）. ROC curve analysis shows that the AUC of PIV was 0.815， the best cutoff value was 355.69， the sensitivity was 78.9%， and the specificity was 84.5%.Conclusion：The incidence of frailty was high in elderly patients with colorectal cancer before operation， and clinical attention should be paid to elderly， female， low BMI and albumin levels， low health literacy， and more obvious colorectal cancer specific symptoms. Meanwhile， the PIV has a good predictive effect on frailty.
