Construction of stratified and graded transitional care program for coronary heart disease after percutaneous coronary intervention

Abstract  Objective：To construct a Triangle stratified and graded transitional care program for discharged patients with coronary heart disease after percutaneous coronary intervention（PCI），optimize the allocation of medical resources，and improve the effect of transitional care.Methods：Based on the theory of Triangle stratified and graded mode，combined with clinical experience，the content analysis method，group discussion and Delphi method were used to construct the Triangle stratified and graded transitional care program for coronary heart disease after PCI.Results：Two rounds of expert consultation were conducted，the positive coefficients of experts were 88.24% and 100.00%，the authority coefficients were 0.89 and 0.92，and the coordination coefficients were 0.401 and 0.504（all P<0.05）.The program included stratification and grading two parts.The stratification content included 3 first⁃level indicators and 21 second⁃level indicators，and the grading transitional content included 3 first⁃level indicators and 42 second⁃level indicators and three⁃level indicators.Conclusions：The Triangle stratification and grading continued nursing scheme for patients with coronary heart disease after PCI is scientific and reliable，which provides a reference for the effective management and nursing services for patients with coronary heart disease after PCI.
